Question regarding names..pls help!

I am a newly graduate from Philippines and I'm a greencard holder. I've already submitted my request and other forms in my school to be mailed directly to the California Board of Nursing. I just got married after graduation. The records that I have to the school (TOR,diploma,etc..) is using my maiden name, but the 3 forms from the California Board of Nursing that I've submitted to the school, I used my married name. My question is, I haven't submitted my application for NCLEX because I dont know if is it ok to use my maiden name instead of my married name because my identification i.d here in California is using my maiden name. and my only legal document to present the change of name is a copy of marriage certificate issued only by the office of civil registrar of city hall of cavite city. i dont have a copy of marriage certificate issued by NSO because the process take about few months.can i use my maiden name in my application although i used my married named for the 3 forms i passed in school, eventually to be mail by board of registered nurse?

You should have kept everything in the same name until you were done with the licensing process. CA is going to require proof of the marriage in order to be able to use the documents that you had submitted with your maiden name. They cannot legally use them without the verication that you are one and the same.

If you would have asked this question before you sent in things, you would have been told to continue to use the maiden name until everything was done and then submit the documentation to have your name changed legally on your nursing license.

You are going to have wasted time getting the verification to them, etc. Unfortunately, you have made things longer for yourself without realizing it.

And for others that are reading here, always complete everything in the name that was used on your school records to make it easier for a BON, and then submit the name change to them with proof of the marriage. Always much easier this way.

it's a civil marriage and the only document i have is the marriage certificate issued by the office of civil registrar,town of cavite city, i think it takes few mos before i can et a copy from NSO. do you think BON will accept it even if not issued by NATIONAL STATISTIC OFFICE?

do you think the BON will accept the marriage certificate that i have even if not issued by NATIONAL STATISTIC OFFICE? it only issued by office of civil registrar at townhall of cavite city.

You may want to consider changing your name on your CA I.D. To do this, go to the nearest SSA office and bring your social security number and marriage certificate issued by NSO or from the municipality were your marriage took place if it is not yet available from the NSO and request for a name change. Then go to DMV and bring the certified true copy of your marriage certificate, greencard and request for a name change on your CA I.D. They will verify if you already went to SSA to have your name changed. Your new I.D. will be sent to you in 2-3 weeks.

You can use the copy issued by the local civil registrar where your marriage took place if a copy from the NSO is not yet available and send a photocopy to the CA BON together with the Change in Name form which can be downloaded from their site.

Also, you may want to consider changing your name on your CA I.D. To do this, go to your nearest SSA office and request for a name change. Bring your social security card bearing your maiden name, certified true copy of marriage certificate (the copy issued by the local civil registry is accepted), your green card and CA I.D. just in case they will ask for identication cards. You will be given a letter saying that you have requested for a change in name on your social security card. The process is quick and they will send you your new SSN bearing your married name in about 10 days. Next, go to DMV to change your name on your CA I.D. Bring your CA I.D, green card and marriage certificate. You don't have to wait for your new SSN to arrive before you can request for a name change at DMV just bring the letter from SSA issued to you if you are in a hurry. Your new CA I.D will be sent to you in about 2-3 weeks.

It is not a point of what is being changed now but the fact that documents were submitted with two different names, chances are that they will be in several files.

This is where the problem comes in. It never gets straightened out immediately when dealing with foreign-trained nurses.

My school sent documents bearing my maiden name while I submitted the application form using my married name. I have included the other form (Change in Name/Address), a copy of my marriage certificate together with the application form, fees, livescan form copy, diploma and a letter of explanation why I do not hold a local license. When I called the CA BRN to check, I found out that they have all my documents in one file, i.e.,my new married name. The person I spoke to also said that processing time for applications for the licensure takes about 16-24 weeks and I was told that they do not require local license.
