   Only the form would be given to the PRC? and nothing requirements needed? - as far as I can recall, you would only give them the form and pay some fees. how about the diploma from the secondary school? i'm the one who will mail it to them via airmail...

   Your school will be responsible to send your forms but it's not for free. You would need to leave them money for mailing fees. :)
