That sounds very honest and professional Lmm0316, I have never thought of disclosing at a later time after the job was offered. I guess if they rescinded after that, it would break discrimination laws.
I usually wait until they ask if I have anything to add, if it doesn't come up in the reason you left your last job part. You can disclose as little or as much as you want.

Yeah, been applying for almost 2 years with little luck. As soon as HPRP is mentioned in interview, I can practically feel the door slam in my face. Dialysis and a few other HPRP friendly places are not so friendly lately either.

I have done this on my own, didn't follow thru with HPRP, license suspended, than I agreed to re-enroll in HPRP than suspension lifted. Pain in the arse, took 6 mos but license active with restrictions, following monitoring agreement, suckin it up.
