    I also received all of the pre-employment information before my interview, as well as a authorization for a background check. I interviewed at EUH. So maybe it is just the preference of the different HR reps.

    From the wording of the email with the questionnaire, it seems like once you return the questionnaire an interview will definitely be scheduled. October 9th cannot get here fast enough!
