I just made it simple and scanned all 4 pages of immuns into one pdf and uploaded that same pdf to all of those categories to cover my ass. I figure they can sort it out if they want to make it complicated and weird in the first place.

So I just finished the fitting sesh. You get to try on scrubs to decide which size to order from Dove, and also the polos/hoodies like Mirna posted. After trying stuff on and buying my patches it took about 30 min. Easy peasy.

Just submitted my application! I decided to skip the extra 2.5 points... it's too scary to think that a technicality with supporting documentation could disqualify the whole thing. I don't think it would've made much of a difference on my application...
