We are reimbursed .55 per mile. We are licensed in 4 counties, basically if the agency accepts the client, we are expected to drive there.

Medicare Charting Guidelines for HH
I agree with the others here, documentation should be completed by end of day. I tell my staff if they do not want to chart at the visit, they have to have it written on paper (we do computer charting) no way you can remember specifics after a long day of visists; and imagine the state doing a ride along and you don't write anything down at the visit. As for a SOC, needs to be completed in 5 days, but some agencies want it to the CM sooner

Dr. orders for OTC meds
I feel very incompetent in even asking this question but had little to no training to home health. Is it required to have dr. orders for items such as tylenol or stool softener in the home health field? Please provide me the regulations that require or not required info is if possible. Lots of debate going on in our office right now.

Fall protocol
The client did fall at home, lives alone. We always do a fall risk, which he is/was a significant risk. Safety measures educated with all visits. client has frequent falls, needed to be in a NH, but refused. PT visits 3 days a week and OT 5 days a week. Aide 2 x week and RN 1x week. He has emergency response necklace that he refuses to use "I don't want to be a bother"

Fall protocol
Hi everyone, does anyone have a fall protocol that they wouldn't mind sharing? We had an incident of an aide discovering a client had fallen with injury. I did not like how it was handled, but we have nothing in place for an incident like this. If no one wants to share, could I have some ideas on what needs to be addressed Thank you so much in advance!

What will electronic charting do to all our man hours
We do electronic charting and about the only thing it saves, is your hand from cramping from all that writing. It might be some faster but not much.

Nurse to patient ratio in ED
I work in a small hospital, in ER we work with one RN and one EMT. At about 4-5 pts I will call the supervisor who then will come to help out. When a true cardiac comes in it is one on one, besides the code team.

Thank you for the ideas, I will certainly add those ideas to my list. I am still interested in additional thoughts on how to actually run the clinic. We have the main building and one nurse, with second one considering the offer.

Start up of a free clinic
I hope someone can help me with this. I am the administrator of a Home Health agency. However, my CEO has asked me to research and start the process of opening a free outreach clinic. This would only be V/S and simple nursing assessment. Lots of education and medication management. I am not finding anything online to help with this. I want a good disclaimer for my RN that will be working the clinic. Any other advice or resources would be extremely welcomed. Thank you in advance

breaking down charts
We do computer charting, but its amazing how much paperwork is still in the office charts. When we breakdown the chart, all Dr. orders, including referrals, F2F, signed 485's, are kept. Pretty much anything with someones signature is kept. H & P along with demographic sheet are also kept. These are all put into a manilla envelope with client name and dates of service written on the outside. Then stored in the office for 1 year. after that it goes to outside storage for 10 years (longer than required) This is probably all over kill as everything with signature has been scanned into computer chart, but I am old school and can not force myself to have a paper with dr. signature on it being shredded! Hope this helps some

When is Dr. order required?
I am needing to verify when a dr. order is required in home health and when its not. We have a client that requested our aide put ace wraps on her LE. the aide placed them and notified the primary nurse that she did this. We have had discussions at length whether this requires a dr. order or not. Can someone point me in the right direction of finding this type of info Thanks in advance for any help Tammy

I don't know the state reg on a rolling bag but would think the same principale would apply, that there needs to be a barrier between bag and surface in the home.
