   I submitted the old form with the new notary page and it was fine. As long as you printed the form that contained the page that you are not restricted from participating in clinicals you are good.

    CrazyforBen- I think you may have done the re-certification one...there is one on there for 4 hours. Blessed- I think Central clinical days are until 3:00, but call to make sure.
