    Lactate

Seen it dozens of times. Sometimes they did, sometimes they didn't. Depends on the reason for the lactate elevation. A postop cardiac case who's hypotensive and hypovolemic is a lot more likely to survive than someone with a dead gut.

Australian nurse wanting to work in America

No, I actually didn't do any OBGYN placements.
  3. Question out of interest for PMHNPs in Pennsylvania (or in general) - does your collaborating physician have to be in the same specialty ie. psychiatry? I checked the PA code and can find this link but nothing about that. http://www.pacodeandbu...

Australian nurse wanting to work in America

So far I have been licensed in Vermont, Pennsylvania and West Virginia without issue.

Pressors with low EF and CHF

You definitely fill the tank before squeezing the tank. In the case of low EF, hypervolemia and hypotension, you need inotropy for increasing contractility - epi, dobutamine (if tolerated by BP), or milrinone (if tolerated by BP), or maybe dopam...

Can an FNP work as First Assist?

Depends on your state's practice act and hospital bylaws. If you mean a hospital OR, then technically no, because FNPs aren't trained or educated to work in acute care at all.

OR RN to NP or PA

Some surgeons want PAs because they do standardized, multiple surgical rotations in school and come out more ready to work in an OR. You will certainly have an easier path to scrub if you are a PA. However, you'll also certainly be able to get a job ...

Australian nurse wanting to work in America

I had 80 hours of each mental health and paeds placement and that was sufficient for CGFNS and my states licensure.
  9. I did my BN at Deakin in mid-1990s and was able to become licensed in Vermont, Pennsylvania and West Virginia so far.

    Student HIPAA violation?

You really need to ask your school, rather than us. Depends on their policies.

Australian nurse wanting to work in America

Yes, I worked in Melbourne ICUs and then came to the US many years ago now. I did it myself without an agency. As it's fairly simple for an Australian citizen to get an E-3 working visa, it's not really worth the hassle to get an agency and be tied t...

What to do when there is limited access and you need to give IV push

This is a horrible idea overall. NEVER push through vasopressor or inotrope ports. I would rather pause sedation and push a drug, or just put another IV in. I can't imagine any ICU preceptor advising to give a push through a levo port.

    DNP Project Idea

Do a pubmed search - is there a basis in the literature that this is a problem? Probably yes. Is there evidence to implement an intervention for this problem? Usually for admission essays, they want you to have an interest in something and be able to...

    DNP Clinicals / Preceptor

Hello, can anyone who has completed a distance DNP program advise who you got to be your onsite preceptor for DNP clinicals? Did you already know someone in your workplace, or contact someone and ask them to precept? How does that work?

Georgia PMHNP Preceptor Needed for Spring 2022

Your schools should provide clinical placements since you are paying them for your education.

    Hiring Manager Question

Guess I know where you went to school 🙂 Worry not about my facility - my hires have worked out well. I have personally observed how certain schools work, how they vet/don't vet their students/preceptors. One didn't even contact me (acting a...

    Hiring Manager Question

I hire NPs. I certainly would not hire people that are educationally unprepared for practice. That would include certain schools. There is a saying "Even a broken clock is right twice a day". Students should be discerning when choosing a program that...

Can Acute Care Adult-Geri NP work in the ER/ ICU?

Age lower limit for AG-ACNP depends on state - in my state, it's over 16. I hire NPs for the ICU and certainly only consider acute care NPs, regardless of nursing experience in the ICU. An ICU nurse who does an FNP program is not educationally o...

    Kindred ICU

ICU is a bad choice if you don't want to work nights - critical care goes 24/7/365. Kindred is not an ICU. It's an LTAC. Their "ICU" patients may have higher care needs but it's not a true ICU.

    New Grad Endocrinology NP

I didn't, but as an experienced APP, endo is very mysterious and complex to me LOL. I will say the experience will be useful whatever you move to after that.

Surgical NPs—Progress Note question adding attending?

Also depends on the payer - some 3rd party payers do not credential APPs in my state, for example, so that gets kicked to the attending for billing. For most patients, I do my own billing.

    Moderate sedation/ pharmacology question

Lorazepam lasts a lot longer, so that policy makes no sense. Raise the question in your workplace.

Switching specialties and leaving ICU- will I be bored?

I'm a supervisor of APPs in an ICU, and this is not usual in any ICU in my institution, or any local ones that I have heard of. I've actually only had to have 1-2 official meetings with someone who was just not improving after a year of training. Oth...
