i'm an lpn in ltc, and i'm the primary care plan writer for all 139 of our residents. an rn may add one or update one i've written, but, for the most part, i'm it. kind of scary...not the lpn part,...

my favorite complaint was from a family member who said "your ice cream melts when it's left out"! maintenance said they could fix that problem, but other patients might not be happy with the room...
help. i had to have my remaining ovary removed saturday night-it had twisted 3 times and was necrotic-incredible pain. anyway, i'm home now and contemplating. i now have no ovarys, uterus or cervix....
amen, suesquatch! never, never, never is anyone "just" an aide. lpn's and rn's couldn't survive without you. a good cna is the heart and soul of a nursing facility. you know your patients better...
In our facility, as long as someone receives more then 26% of their calories from tube feeding, even if they're stable, they remain on Medicare A, and after all 100 days are used, they remain skilled....
Thanks for the suggestion! I always keep a look out for significant change, but sometimes it's just that the pt needs therapy for a new type of splint or family is requesting a dietary upgrade, and...
Just curious....has anyone ever heard of doing an "early" annual? Not doing 3 quarterlies between annuals? We searched the RAI manual, and it only says that an annual must be done AT LEAST every...
I've been doing this for three years. In Pennsylvania, only full assessments count towards the CMI. I don't know how it works in other states. You're right...the RAPS are certainly time consuming,...
Holy Cow.....that's WONDERFUL staffing!! We staff 2 nurses and 6 CNA's for 48 patients on day shift. When I worked 11-7 there were nights that I had 75 pts and 3 CNA's to help. Those were nights I...
I have mixed feelings on this. Yes, I think we can get rid of things like cholesterol meds and BP meds for borderline hypertension, but I see many pts do poorly when we try to decrease their...
thank goodness....you were thinking exactly what i was!! heck, i don't want the doc to "come".....i just want orders. he's going to come once a week as usual...if he thinks the pt needs seen sooner,...
I just can't figure out how 3.0 is going to calculate new CMI's. So much is different. I mean, skilled people could have the same RUGs, but what about our Medicaid people....all those things that...
Wow...it's nice to see that there are other MDS nurses on this site!! I've been the "RNAC assistant" (I'm an LPN till 2009)at my facility for 3 years, and our CMI has risen from 1.14 to 1.50, But,...
:smiley_ab Hang in there. It's SO hard on a relationship to be in school and working. I'm doing it right now and have had several arguements with my s/o. Give him some space and see how things go....
i'd go right for the rn. it doesn't have to be a bsn, though. if you go for an adn or diploma rn, then you can take a few classes at a time to get your bsn (if you like) and still make decent money...
Hi! I know this was originally posted a while ago, but thought I'd give my $.02 worth. I think it's sad that the LPN schools are marketing so aggressively and charging outrageous prices. My DON said...
I've seen this a few times and don't understand it. Why aren't some of you allowed to give a PRN med without approval from an RN?? I mean, the MD/DO order is already there, and you can certainly...
[quote name= Sorry you are having to see what much of the LTC in this country is really like.[/quote] Oh gosh, now I just have to comment......I don't know how in the world "much of the LTC in this...
I hate to say it, but on the rare occasion that I pass meds (did that for 15 of my 18 years as an LPN), I might take some shortcuts. I start my med pass a little early, I've let experienced CNA's do a...
I don't have quite the same scenario, but I understand your concern. I work in LTC and we have some 90 year old pts who are full codes. When I do the initial care plan meeting (yes, I'm an LPN who...
It's got to be tough right now. I talked to our DON (i work in a nursing home) just the other day and she said she can't hire any more LPN's for any shift because they're full. Heck, she's...
I'm sorry...I should have been more specific. I don't have to get approval, but I've seen more then a few posts (I think in the forums where people were asking the difference between RNs and LPNs)...
i totally agree with you.....i had to change the way i think and forget what i know. i have to give the "book" answers, not reality!! when the question states "what would you do first?" i'm...
