    We have no IVs, wound vacs, complicated treatments, or catheters. We have 5-6 treatments to be done daily. The ADON and I are constantly asking them if they need help, what can we do, what are your ideas. They never think we have our work too. The ADON and I do all the admissions, completely. We have 5-6 aides on days, 4-5 on evenings, and 2 on nights. I have meetings to work on solutions to our problems. A few of them will give input. We do have approximately 50 % of mod care residents. The aides are great! They do want ever I ask of them without complaining.

    Nursing staffing

    Our nursing home has 42 residents. My ADON and myself are always available to help the nurses. We just ask them to let us know when they need help. Many times we offer. The staffing is 2 day nurses, 2 evening nurses, and one night nurse plus a sufficient supply of aides. They always complain that they don't have enough time to do their jobs. I think they are lucky to have the staffing they do. I took over the DON position one year ago. I know some of the older nurses are not willing to be involved in a "team environment". The old DON has been working as a staff nurse. I don't have these issues with the aides. They are great. We are working on "team building"and things to help improve their moral. I don't micro manage and encourage and initiate their ideas. Any ideas? How's your staff if you are a home similar to our size. Thanks for your input.

    Control of controlled medications

    I hope some one can help me. New guideliness for the counting of controlled substances came out where you have to count more of the controlled substances. We've only been counting Schedule II. I think you're supposed to do one week out of the month of counting other schedules. How have you been doing this? Any suggestions on the best time, which shift,etc. Thank you so much for your help!

    I want to thank everyone who replied. It's very much appreciated. I'm sure we'll get along fine and work together . I think we both need a better understanding of each of our jobs! Thanks again.

    She usually has 3-5 assessments. As I mentioned earlier the nurses do all the assessments, Braden, Fall, etc of each new admission.

    She's only been doing them since March. She is doing a thorough job on them. Care plans last about an hour. I'm impressed how she has caught on. And she is very detailed. I've only been at the facility for a year and had to redo every system when I started. I'm probably feeling overwhelmed as she is.

    I should add that we actually have 57 beds but always run around 40-45- beds. As DON I also have a 28 assisted living units.

    I'm a DON at a 40 bed facility. When I was hired I was told I would have assistance from the ADON/MDS coordinator. Currently she does 4-5 MDS a week. I've been told that an admission MDS takes 5 hours and others take 4 hours. The charge nurses do all the assessments for all admissions. She also does Infection Control, arranges inservices, updates orders on the MARS. She also runs off the certifications and recertifications every 60 days. We both take turns working 3 hours a week and share the times we have to fill in for other staff. It's seldom that we have to fill in. I want to be fair to her. She does nothing else to help me. I'm overloaded. She is also a good friend of the administrator which is easily seen in the work place. She feels she has too much to do and gets extra help from another nurse to do her assessments. Is this load too much to handle? I want to be fair with her. Thanks for your input. I really appreciate it.
  12. This is the situation. The ADON who is an LPN is waiting to become the DON. She has completed RN nursing school and has not passed her boards after three tries. A RN day charge nurse has been told that the facility is using her license to cover the facility until the ADON passes her boards. This doesn't sound right to me. The RN has not agreed to anything. This is for a skilled-ICF facility in Iowa. I would appreciate your input on this situation. Thank you very much.

    DON and filling in for staff

    1. As a DON of LTC how often do you have to be the "company pool" for staff who call in. Right now I"m charge nurse for 4.5 hours per week not counting the call ins. I'm new to this position and have had to start from scratch since the facility was so non-compliant with state and federal regulations so I have alot on my plate. What is the average time you work the floor ?

    Thanks for your input. It is in a small town and run by a private owner. I'm not sure about the night yet, who who fill in. They say they have extra staffing otherwise the ADON and me would fill in.

    I've been offerred a DON position at a a 57 bed facility which only has 37 residents. The Administrator has been there 14 years and does the staffing schedule. The staffing is as follows for 37 residents: The DON and ADON are not counted below. Days 2 nurses with 5-6 aides Evening 2 nurses with 3-4 aides Night One nurse with 2 aides. The ADON also does the MDS. The facility has had very few deficiencies. The Administrator and I both have strong personalities but have mutual goals. The staff appear and act professionally. Please give me any insights. What do you think? How is the staffing? Would you consider a building with these staffing ratios? Thanks so much for your input! I really appreciate it.

    Corporate headaches

    I request the following: Skin log Fall /Incident Log Dietician's reports I review all of these to assure we are doing everything we can for the residents. I report to the owner who wants an update weekly.
