I reached out via phone directly to the Administrative Coordinator (MODERATOR EDIT OF INITIALS) and got the acceptance form. So, if anyone didn't get it, try that way. She picked the phone up immediately and sent it via email right away.

TEAS 87, Overall GPA 3.6 (much earned from previous BA and lemme tell ya it was years of work getting it up). However, everything earned since then has been a 4.0 with a Science GPA of 4.0 without any withdrawal...
