    They will accept it with Pharmacist signature. I didn't have a name, just the signature and they accepted the vaccines.

    You would have to ask them, but probably not. They are strict with deadlines. Was it Hep B?

    I understand the requirements... titers are acceptable for all except flu and tdap. My PCP recommended a booster for all. I attached information on Hep B booster. Read bottom right section.

    Did you get vaccines as a child? I don't have my records but my Mom assured me that I did. It's common for levels to be non-immune on titers if you got the vaccination 20+ years ago.... My PCP advised me to just get a booster and then wait 4-6 weeks...
