Not sure haven't made an appt yet with my doctor. I'm sure the form will be available once we get accepted maybe you can drop off so Dr can fill afterwards?

Also RE: the medical forms. I would email the programs and mention "for financial reasons it would be difficult to pay for multiple instances of the same appointment/ to have the forms filled out at different times.
*since its school/occupational some insurance doesn't cover it, so its not a fib.
I would also ask them if they will accept physical documents from another area program. SD city college has them in the nursing handbook and southwest has them in their application (and I'm sure grossmont has a very thorough form. LOL)

I have not heard back from either one but knew I had to get on it for grossmont. The wait time for physicals in my area is months. And you're right, it's costly to repeat an appointment if labs etc are done to measure health (or for titers etc). The problem for me is that Grossmont's orientation is in early June and I need everything done by then. If classes start in august for palomar, waiting until mid or late May (when acceptances are sent out) to begin the process of making an appointment and getting all that done before palomar's orientation seems to be cutting it close! Then there's the BLS course, the malpractice insurance, and all the other little things needed for onboarding. I'm hoping Palomar gives their applicants enough time to get everything done!

That is a very thoughtful answer that makes a lot of sense. I was talking to a nurse yesterday while I was at urgent care for my daughter and the nurse was commenting on how difficult a task it was for nursing students during Covid. You're absolutely right that the scores I mentioned reflect that.
