pain management in infants with chest tubes

How do you manage pain in infants with chest tubes? I have an infant on nasal cannula now, after being vented, but has had a chest tube for four days now. We gave fentanyl prn when intubated, but now when asked for some kind of pain medication the physician, the physicians say baby is irritable because he is hungry. I agree tha's part of it, but I think pain is a big part of it.

Holy **** (insert choice word here) Elizabells.... I can't believe that you had to fight so hard for TYLENOL. When we put the orders for a circ in, part of the orderset is for tylenol q8 prn for up to 48 hours. It automatically gets ordered. It's hard to believe that even getting hit hard by JACHO hasn't started some change for the better. Maybe it's time to go to hospital management about this problem - somewhere higher up than your unit manager.
