So not for nothing, but have you ever had a consult doc write orders, place them into the middle of the chart and then put the chart back on the rack. Not in the slot where it flags that orders have been placed, but down on the rack. Turned out that the orders weren't even complete. Not sure why this doc didn't use the computer system to place the orders, he's usually pretty good. Yeesh.

Yes, Doc's make mistakes, but usually it is the Nurse that get in $#!% for the mistake. As long as he owned up to the mistake and did not blame the nursing staff.

What was the upshot of this really bad boo boo?
Have you ever seen a doctor go back and alter his or her orders after a nurse has already signed them off? Imagine the repercussions that could have.
