For number 17 it says to list all schools you attended. It's asking for the exact dates (month, date, year). Does it matter if I don't remember the exact date? Last year i applied for the LPN and I can't find my copy that I made because it has the dates on it.

The month/year should be sufficient.
Note to self: this is the type of information you'll need again and again for resumes, job applications, licensure applications, etc. Gather it in one place and keep it.
Ok, so I can just leave the "day" line blank on the application?

Just to clear things up, there is no such thing as an NCLEX application, you will not find one anywhere. You are applying for licensure as a nurse to a state BON and in your case, it is NY. You need to have permission from a state to even sit for the exam.

is there any way you can contact the schools and find out when terms began, ended, or graduations were held? i doubt as well that the exact date is important but i hate leaving things blank!
