Nursing Diagnoses for equilibrium

My instructor hinted that I should do equilibrium as need for because my pt has septic shock, infection, and a UTI. This is the first time I have had to write a care plan for equilibrium and I am having a hard time linking this info to a diagnoses. If someone could please guide me in the right direction I would appreciate it. I have the Ackley book but it doesn't give me a diagnoses that I can come up with.

I don't understand your question completely. Your instructor wanting you to use 'equilibrium' in your care plan is throwing me off a bit.

When I read what your pt's problems were I thought of ~ infection ~ impaired tissue profussion ~ pain ~ risk for injury

Are you referring to balance? As in electrolyte balance?

When doing any care plan, your first step is to always start by listing out all the abnormal symptoms your patient has. Certain abnormal symptoms are then grouped together to form the basis for specific nursing diagnoses. These are obtained from your data gathering and physical assessment. If the patient has an equilibrium (or balance?) problem it will show up in the symptoms you have collected. It sounds like your instructor feels there was something about the patient to make this assumption on her part. Is it possible you missed something in your assessment that she might have picked up on that led her to suggest this way to go with a care plan?
