Nursing Care Plan Quadriplegic Patient PLEASE HELP

I picked Self-Care Deficit for my NANDA my patient is quadriplegic and can't do anything for himself I'm having problems with my goals and nursing interventions. If anyone recommends another NANDA please do.

Self care deficit: bathing, feeding, dressing R/T neuromuscular impairment

AEB: inability to get food onto utensil, wash body, put clothing on upper or lower body

Goals

Short Term: NEED A SMART GOAL

Long Term: NEED A SMART GOAL

Nursing Interventions:

Assess degree of disability

Rationale: Will help establish a plan of self care.

Assist with meeting patients self care needs

Rationale: Personal care is part of nursing care.

Assist patient with rehabilitation programs

Rationale: To enhance client's capabilities and promote independence.

Provide for communication among those who are involved in caring for or assisting the client.

Rationale: Enhances coordination and continuity of care

Consult rehabilitation professionals for proper mobility or assitive devices

Rationale: Modified eating utensils or modified clothing might be needed for proper self care.

Teach ???
