I am in OB and am struggling with care plans. We have two, and I mean Two care plans due next week on top of exams and theory and clinicals, I don't know how I am going to find time to do all of this.
Cut to the chase:
So my PP mom has a bleeding disorder. My nursing diagnosis is Risk for bleeding R/T inherited bleeding disorder. That doesn't sound right to me. Or maybe Risk for fluid volume deficit R/T inherited bleeding disorder? I'm on my second care plan 8 hours later, so sorry if I don't make any sense. Please HELP!!

OK, let's stop and take a deep breath here.
"Risk for bleeding" is a perfectly good nursing diagnosis to make if your assessment identified at least one approved risk factor (it sounds as if you have: a coagulopathy). It's on p. 382. (If you don't have the NANDA-I 2015-2017, stop immediately, go to Amazon.com, and get this book for immediate download to your iPad or Kindle, or overnight delivery in hard copy. I'll wait.) Its definition is, "Vulnerable to a decrease in blood volume, which may compromise health."
What would you, the nurse who is prescribing and delivering/delegating care, want to do with someone who is at risk for bleeding?

Seriously, you need to have a NANDA book on hand. We cannot do your homework for you. AliNajaCat has given you some good suggestions to get you started. Also, if you're in a study group it might be beneficial to get everyone together and brainstorm. No, this isn't an easy assignment, but you should make it a priority. Good luck to you.

I am not asking you to do my homework for me. If that was the case, I'd post the entire care plan sheet. If you had read, I am on my 2nd care plan in 8 hours and I may not make sense. Just asking for a little help since my brain is fried. Thought this was the place to get a little help, not tell someone to do my hw for me. Thanks for the input.

Wow. This actually is a great place for homework help, in the student section. But the attitude needs to go away. You have had more than one very experienced nurse give you hints tonight. Go with it.

And yes, we've all been there, done that. Most of us without benefit of the world-wide internet. So buckle down, take the advice that you asked for and go buy your NANDA book.
