Number of Disease Management nurses

I was wondering if there are many of us in the club? I rarely have seen postings from DM nurses so I wanted to do a head count.

If you just post you are one that would be great!

did you mean 40 a day or week? because if you work 40 hours a week thats only 8 a day. i previously worked telephone triage and took 4-5 calls an houir. h

yes - 40 per week. disease management is different from telephone triage. each call is around 30-45 minutes in length with goal setting, discussing barriers and strategies to meeting the goals and differing levels of education depending on what the patient needs. also, since most of the calls are outbound, you have no guarantee that the patient will be home when you call. i usually call between 15-25 per day , and usually only reach 5-7 of those .

what company do or did you work for disease management? what type of case management are you moving into?

do some call you back also? if you did not reach them the first time? what area of disease management do you or did work in?

I work for a large DM firm. We are expected to do 4 calls an hour - 10 minute calls and 5 minute wrap-time. Most of my patients are seniors and it takes some of them 10 minutes to find their medication list. Very frustrating, I don't feel like I am doing quality work at all, it's al about numbers and productivity.

do you mind telling me what type of disease management you do and for what company? 4 calls an hour could be considered alot. i do that in my triage work but now am starting disease management and i know some of my calls will be 30 minutes or longer.

Yes - we leave a message and they call back --- sometimes. There doesn't seem to be alot of incentive for patients to be involved in the program. Either the patient is willing to make the necessary changes to improve their health, and they don't really need us , or they are not willing to listen and make changes, and those are the ones that never call back, or act like you are boring them to death on the phone when you talk to them.

Do you offer them a free meter? I wish they had asthma where I work that is where most of my experience is in. It appears many people do want to participate in our program.

Question...did any of you have to move to take these positions, or can you set up at home wherever you happen to live? I love the job I have now, but with gas prices being what they are, I would sure save alot working at home.

Do you have to be based in a particular area? Or will they let you work from whever you happen to already live? I live in SW Mo and so far haven't seen too many places that work from MO.

I am still hanging in there with DM and like it alot. My caseload is higher than others mentioned and call durations are much shorter.
