New Mexico - National Screening Information

Published

hi allnurses,

i am in the philippines. i would like to seek help from you since i am now applying for new mexico state board of nursing.

my concern is about the the forms. please click this site where my concern is all about.

http://www.bon.state.nm.us/forms/background_ck_and_fprint_instructions.pdf

page 1- nationwide criminal history screening

page 2 - fingerprint certificate form

page 3 - authorization for release of information

since this form needs to be notarized... my lawyer is asking for assurance before this document becomes legal. we try to prevent technicalities on this matter and i am afraid of any errors since this is a legal docs.

can we notarized it here in the philippines?

another thing is i was trying to call the bon nm with this tel no. (505) 841-8340 but i cannot get into their lines and it says " this call is not permitted"...

how do we go about this? please help me.

First, you are calling from a line that allows for international calling? Next, you are using the proper country code to make the call? United States is +1. In addition, the NM BON has a voice system, this may cause problems depending on the type of phone you are using.

Specializes in Med-Surg, Peds, Ortho, LTC and MORE.

New Mexico just changed the areas code to 575 in some places try that with the rest of the number you have that also might be why you can not get through.

The area code remains 505 in the Albuquerque area.

In the many times that I have called their office, I had only one chance to speak to an actual person. Usually when I call, an answering machine greets me. But the nice thing there is if you leave your name and contact details, they are prompt in returning your call and answering your questions.
