I am looking for 2 things. First, working in med/surg, I'm wondering if anyone has a form they use for important information when getting & giving report...a little "notes cheat sheet" type of form. Also, looking for a listing of most common meds given for particular diagnosis (like blood pressure, cholesterol, anxiety, etc.) so that I can better recognize the names of these meds and what they are for. Kind of a new nurse...and still trying to make things better.

google "nurse report sheets" and you will find a plethora of choices! The one I use is a compilation of several I have seen over time

for meds for a particular diagnosis, look in your drug book...either in the table of contents, or in the index, drugs are likely to be listed by diagnosis they're used for...

There are excellent lists of common drugs and their uses on the student section on this website. Look for the sticky on random fact throwing. It's for people studying for the nclex........they really are good!!!!

When I get report I write EVERYTHING DOWN I mean EVERYTHING. I also write a "to do list on the back" as I'm getting report. As soon as I find out something needs to be done etc or you did something I write it on my sheet. Usually I have a BM section, follow up section, new orders section, current situation section, and for me any PRN's given as my facility demands that.

This is a copy of the form I designed for myself. I clip it to one of those clipboards that has storage in it for extra forms, post it notes, paper pads and extra pens. I use a yellow highlighter to highlight the things I need to make note of.
