Need help with Wound Assessment question

HI I AM STUDENT NURSE FIRST YEAR,

MY FIRST ASSIGNMENT IS FOR A WOUND (skin tear) to an elderly lady, my first question I am really stuck on is as below

1. Describe what would be your initial assessments and/or actions to the wound?

I AM NOT SURE WHAT THE TEACHER IS LOOKING FOR AND SHE IS NOT VERY HELPFUL, CAN ANYONE PLEASE HELP ME TO GET STARTED, I AM ALSO NOT SURE OF WHAT OBS TO LOOK FOR, NUEROVASCULAR OR NUEROMUSCLAR, NOT SURE IF THE CLIENT HAS HAD A FALL

ALL THE ASSESSMENT SHOWS IS A SKIN TEAR TO HER LEFT ARM WHICH CAN BE REALIGNED,

BUT I AM TOTALLY LOST - PLEASE HELP ANYONE

Think about it. If you found a wound on someone, what is information you would need to describe it to someone else?

This question has nothing to do with a fall so neuro checks aren't necessary. You are simply asked to assess the wound. Don't read more into a question than is actually there.

Now knowing what you should know about wound assessments, describe the wound. Location, size, color, drainage, odor. How would you document the wound? How would you clean it?

I don't know how much you've learned about wound dressings. So if the skin tear can be realigned, what would be used to hold the skin edges together?

Hi thanks but the teacher wants us to look at the patient as a whole, she want us to follow star, we have never seen a wound let alone dress one?
