It is my understanding that someone on an H4 visa is prohibited from working. Under certain circumstances those on student visas can work limited hours. Have you tried posting and/or searching in the International nursing forum?

I would post my services online or a local newspaper. Perhaps state you are currently in Nsg School and are willing to provide inhome care services to the elderly or disabled on a private basis. That way you can establish yourself both as an independant contractor or 'family memember.'

H4 you can not work, may be different if on F1 but H4 you have no chance

You'd need to change to a student visa in order to work, and even then you'll be limited (Not sure, but I think you can work on-campus only?)

This would be a violation of immigration law and, if you're also suggesting the OP work "under the table" and not declare the income or pay taxes on it, a violation of Federal and state tax laws.
