Need help with declaratory order for Texas BNE

Ive been accepted to LVN program and am having to file a declaratory order for the Texas BNE. I have had trouble paying my traffic tickets over the past few years and have had a few failure to appear charges, bench warrants, & my license has been denied renewal. I know i have to get documents from DPS regarding my license renewal, but how will i know what the "supporting charges" are? Do i need to report warrants if i was never arrested and have paid those tickets? I also have a conspiracy to theft charge from when i was 14, it was deffered but i am still unclear if that needs to be reported? & my last question is, do i need to be worried about my eligibility? I am a very poor single mom, im only 20 years old and had no idea that i would face this issue going into nursing school. Any answers or input would be much appreciated. Thank you
