Hi there! I am a new RN, starting out in med-surg. Does anyone have any report sheets or patient organization sheets that you could pass on to me? Any help would be greatly appreciated, as I am still trying to figure out what works for me...
I was in the Float Pool for many years so I had a generic report receiving/change-of-shift. I would do a brief run/down of all systems:
I would just put the letters that corresponded with the system:
N(neuro)--are they alert/oriented, any behavioral problems
P(pulmonary)-any breathing problems, are they on O2, RT treatments
C(cardiac)-Heart rhythm problems, chest pain, blood pressure issues
GI/GU-are they voiding or have a Foley, any constipation problems, diarrhea, blood in their stools.
Pain-any pain issues and when they last received analgesics
IV-site, rate, IV gtts/medications
Diet-are they eating, NPO
Activity-bedrest, mobility issues, fall precautions or any other precautions
It goes pretty fast once you get a rhythm going.
I Just Searched And Found All These Report Sheets. They Are Excellent For My Assignment Due Tomorrow. They Will Also Be Helpful With My Preceptorship Starting Mon. Thanks For The Help!!

Yes. I've been making my own report sheets for years.
Thanks for the sheets, starting my preceptorship this week in the ICU! Graduating in May!!!

Which specific attachments posted by which members are you referring to? I am e-mailing my report sheet to you, but the moderators are going to remove your e-mail address from your post as we are not allowed to post them publicly.
