You need to take a NCLEX review it sounds like and maybe cut back on the hours you work. Where are you located? There are several reviews out there that you can take.

Mucous Membranes..... Do you say dry and white if some poor oral care or dehydration? You say moist and pink if they are ok. I am trying to tie this all together.

I am a nursing student, finishing up my last quarter of ADN, ( LPN now) I have read in several different charts of one dictation saying that a J tube was inserted, and read in another dictation that a Gtube was inserted. Is there a sure fire way to ensure which is which? Jpegs are higher on the anterior aspect of the abd, correct?

I am pretty confident in my head to toe assessments, however, when I find somthing is wrong, I am not sure how to document it in proper nursing terms. Example: Head is symmetrical, but dandruff noted? Eyes, scalera is white without pigmentation, pupils, equal and reactive to light? M. M., dry and white? How do you write the problem in proper nursing terms? Anyone please help with this. I am really confused and struggling. I want to be good and not miss anything that should be documented.

Your skills may be up to par, however, two different scopes of practice. I would be afraid to challenge the RN-NCLEX without nursing classes. As a RN you need to know your scope.

My LPN clinicals were all hospital except for the first semester.

Oh, I didnt work there, I just thought I was at work because I was in a healthcare setting. Apparently I felt I needed to clean up my work area. Versed a wonderful drug, dont remember it at all. As I am told, I preheated a oven, cooked a pizza and ate it. News to me...Learning first hand the reason why you should have someone with you at least 12 hours after such a procedure.

I had a EGD a couple weeks ago. Demerol and Versed were administered. I woke up with N&V ( Narcs make me sick) So, got a dose of phenergan. Apparentally in my drug induced state, I thought that I was at work. I pushed the nurses hand out of the way when she was trying to remove my IV. I announced to her that I was a nurse too and I was more then qualified to remove the IV. Once that was over, I was awoken to get dressed and ready for discharged. I got dressed and then I started to "clean up" Removing the bed linens, trash, ec..... I was totally unaware that I did that. My husband just told me. I am so embarrassed..... OMG>... Has that happened to anyone else? Forget that they are the patient and give health professionals a hard time?

I feel it would be very confusing for someone in a hospital setting to distinguish between RN, LPN, Tech... In the hospitals in my region, they are having the RN's wear a RED badge a little longer in length than the photo ID, it says RN. A patients sister said that her mother was in MEDICAL school. I said really? What field is she going to go into, pt replied a MEDICAL ASSISTANT! I actually had to think a minute to be sure I understood that I heard what I heard. My providers office also hires an abundance of MA's. I needed to have a medication refilled and received a cell phone call from a women that identified herself as Dr"s nurse. She asked what my allergies where and If I was on any other medication. ( my file was in another office) I told her I take one 0.125 mcg of levothyroxine q morning..... She asked me how to spell levothyroxine

I can not remember if it is heparin or lovenox, but they are now coming in premeasured syringes with a air bubble, the air bubble in the syringe act's almost like a Z track method. It is not supposed to be pushed out, and the bubble helps with the concurrent hematoma seen with heparin or lovenox injections.

Well, that is scary! No scope of practice. I didnt think that anyone could practice under anyone's licensure... Hmmm good research project.

I have also heard of baroreceptor therapy. Instructors at my school said that they heard of it and from what they heard is you have the therapy and the habit is gone. I wonder if it is worth a try.

I have noticed in the last year a decline in my resps status. I am starting to sound like a smoker... I hate that. I smoke when I study, drive ect... It is like my best friend... How sad is that? My nine year old is learning about smoking in school, how to say no! He comes home with new education for me everyday. I need to stop not only for me, but for him. I am going to work on it. How do you get away from the habit? Does anyone have helpful hints on the social aspect's of the habit?
