Need end-of-shift report sheet!

Hi there! I am a new RN, starting out in med-surg. Does anyone have any report sheets or patient organization sheets that you could pass on to me? Any help would be greatly appreciated, as I am still trying to figure out what works for me...

Mine's a little bit of overkill, I'm sure, but I'm a new nurse, so I like having the "extra protection" of as much data as possible. It's designed to be front and back, and perfect for a clipboard. The problem is.... how do I post it on here? I don't see an attachment option other than link. Did I miss something?
