ve made huge medication error, unfortunately my num is not supportive she always looks for the bad and acts on it. But never praises the good.
I have had 2 what i would class a minor med errors before, but i was not given a warning i was given a formal warning which is on my re ord for 1 year per error.
I had a call today to say i have another formal warning next week in front of my num and my dcs.
I am terrified that i will get the sack.
Can anyone out there give me some advice on what to say in my meeting. I am absolutaly devastated by my mistake and thankfully the patient did not hace any adverse affects from the 12mg instead of 6mg of hydromorph tablets. P,ease help me.

You double dosed a patient on a class 2 narcotic? Did you self report or was it caught during narcotic reconciliation?
Call your malpractice carrier ASAP for advice.
You have very specific details, if this is your real name change it ASAP
