Legally, do you have to do Medicare A, B, C, D charting Q shift or daily for Tennessee?

Since Medicare is a FEDERALLY regulated program, the rules are the same state to state.

Med A requires (at least) daily documentation. Most places document every shift.

Med B requires a weekly note which should address the reason the person is being seen under their Med B benefit (slp 3x weekly for safe swallowing strategies) and how they are progressing.

Med C and D cover only pharmacy expenses and do not require any charting as such.

I I work long term care and medicare charting is done once a shift we use flow sheet with everything on it.
