Making care plan for current treatment

I am curious if it would be appropriate to do a care plan for some problem that is being treated. For example, if a patient is on multiple laxatives then there must have been a constipation issue. The patient may current have daily soft stool due to the laxative use. Can I still tell my students that they can use constipation as a nursing diagnosis and do a care plan?

Our school. The first nursing diagnosis has to be about the chief complaint like say the patient is in there for Cellulitis...then the nurse diagnosis has to be something like impaired skin integrity and then do the plan of care.

The 2nd one could be secondary problem like if the client was dealing with constipation all day....and if that is in our nursing notes of something they had been dealing with ...then we can do a plan of care for that one.

I'm just student though but that is what our school requires.
