License re-activation and DUI

I am in the process of re-activating my LPN license. I haven't practiced nursing in 15 years but kept it active until 3 years ago. I want to go back to school to earn my RN and when I recieved the form for re-act. there is now a questions concerning DUI convictions. Between 1994 and 2000, I had 4, the last one being 11 years ago. That is a closed chapter in a bad patch of my life. Does anyone know if these will prevent me from getting my license back? I want so much to return to school. Help anyone?

Please tell us what state you are in. I'm curious since even one misdemeanor DUI in here is keeping people from any license. Also, did you change states, or have you been in the same state the enitire time. Also, what type of evaluation process were you ordered to take part in prior to reapplying?

I have yet to see a negative post. I have seen the majority of opinions that this scenario is or was highly unlikely to have a positive outcome, based on an awful lot of personal experiences posted in this site.
