June06 License Waiver

Hello to everyone... I failed the retake of test 3 and 5:o But I passed my nclex just a week after knowing the result of the retake. I am planning to retake the whole NLE test 1-5 this december 2007. Can anyone please help with the format of the waiver for the license? Thanks!

you could go to prc, they have a format there.

Hey Dudzy, Do you happen to know when is the deadline of the retakers for the December 07 NLE? Btw, congrats for passing the NCLEX :balloons::balloons: I haven't found out about mine yet. I took it last week. Congrats again!:balloons::balloons::balloons:

Thanks! I don't know when exactly is the deadline but i'll file this week... SO i can set my mind that I will definitely be retaking the whole exam this december07...:o

I have a copy of the affidavit of waiver. For those who need it, PM me.

To diana, I believe you have the same schedule as the regular retakers to register for the Dec 2007 NLE, that is, Sept 17-28. The same thing was held true when I retook the entire exam last June 2007 NLE.

Hey my friend told me that she read in the newspaper that the start for filing of the retakers will be on the 17th until the 28th of September!

When I handed my plastic license to that cranky clerk behind the desk, I was steaming mad. That was the most absurd thing I have ever done. Imagine waiving your prized license?? :angryfire

I ALREADY WAIVED MY LICENSE TODAY!!!!!!!!!!!!!!!!!!!!!!!!!! I am no longer an RN here in PI. :crying2::sniff:

The requirements i submitted were the following:

1. Board certificate (surrendered) - original and 3 photocopies of it

2. PRC card (surrendered) - and 2 photocopies

3. Board Rating (the one sent to me by mail) - original and 2 photocopies

4. Affidavit of waiver - 2 originals and 1 photocopy

5. Community Tax Certificate - present original and 2 copies

6. TOR - present original and 1 photocopy of whole set

7. NSO - present original and 3 photocopies

8. passport size pix w/ nametag 4 pcs

9. Fill-up forms

10. 900 pesos for exam fee

Thats all that I submitted.... Hope this info can help those who will waive their licenses...
