Give dobutamine 500mg in 250ml D5W at5mcg/kg/min, wt = 242lbs.
What is IV rate in ml/hr and give a H and V that will double this drip rate.
I know how to find rate ml/hr and got 16.5ml/hr but I dont understand how to find out second part for H and V.
I found 16.5 by doing 110kgx5mcg/1kgx1min = 550mcg/min
Then used proportion to get 33,000mch/hr
And used DA 33,000mcg/1hrx250ml/500,000mcg = 16.5ml/hr
I have no idea how to find second part of problem

An RN told me today that I would just change the 5mcg to 10mcg and not the 500mg or 250ml because that's "as it comes" so that confused me more.
She said that because Dopamine usually comes pre-mixed from the manufacturer. However, the standard concentrations seem to be 400mg/250ml and 800mg/250ml. The higher concentration is often used when you need to reduce total volume of all fluids being infused.

Are we confusing Dopamine with Dobutamine?
Is there no way to figure out the new on hand and or volume given the rate? I guess no one thinks so since the post has had over 100 views and no replies.
This is a bad question. You are NEVER going to need to calculate what's on-hand. You get what's put in the Pyxis for you. Typically, there are only 2 concentrations to choose from and that's already been pre-determined by the pharmacy before it's sent up to the floor.
However, if you want to know how many mg/mL, then you can calculate that out using dimensional analysis. Start with your desired drip rate (double what you calculated) and work your way backwards. Or, in other words, how many mg/mL will give you the drip rate you want given your patient's weight?

No, in my experience the pre mixed Dobutamine comes as 250mg/250ml or 500mg/250ml. I checked with our Pharmacy and several of my former ICU workmates and they agree the pre mix is ALWAYS 250mg/250ml. For higher ranges, they request the 500mg/250ml from Pharmacy.
At my current hospital, Dopamine is always 400mg/250ml D5W. A higher concentration would have to be a custom mix by Pharmacy.

No worries. I've never worked with the concentrations you have. And like you I checked our pharmacy and called a friend from two of my previous jobs to double check and I was correct. Maybe it's a regional thing.

As would anyone. You prof is not making herself clear. When she does, let us know.
I, for one, am becoming more disillusioned with the quality of these so-called nursing instructors. A recent thread about nursing students really put the Clinical Instructors in a very bad light. Abandoning their students, sitting in the break room, leaving the clinical facility, dumping their students onto the staff nurses, etc.
They are NOT getting a quality education that I imagine they are paying dearly for.
