i graduated last oct 2006 and i passed the board exam. im planning to take my nclex this year but where can i get the application form? im planning to go to arizona. help me pls.. what website can i download the form? thanks

Please take the time to read the Primer at the top of this forum for information that you need.

You will be applying for licensure to Arizona as an RN, you do not apply to take the NCLEX exam. You are given permission to sit for the exam once your credentials have been approved by the state Board of Nursing.

hi. i also applied in Arizona. u can go to their website www.azbn.gov and download the Licensure by Examination Application form for Nurses. Fill that up and send it the Arizona BON. They will send you after a couple of weeks a fingerprint card which u need to take to the NBI and a candidate bulletin to be submitted to NCSBN. After around a month of filing ur fingerprint card and candidate bulletin, u will be eligible to take NCLEX. They will send ur ATT via email and u can schedule for the exam thru pearsonvue. you still need CES from CGFNS before they will give you ATT

That is why it is only a month or so, you have to have the CES done first before they will even look at the application for licensure.
