Can w.e start a detail study of infection control. This is about a great part of the test we can incorporate availiable information here and add to . Please help

i guess we can share questions on this too, keep it rolling

a client's wound has eviscerated. which of the following will you implement first?

1. stay with the client and notify the physicain

2. apply a clean, dry dressing to the wound.

3.cleanse the would with betadine and apply bacteriostatic oitment

4. place sterile towels socked in sterile saline over the wound.

what would you chose, on the Nclex

I would choose 1. You wouldn't put a dry dressing on it. Sterile soaked towel? I don't think so. Soaked saline dressing is the answer but it's not there so this is tricky.

I stick w/ Saunders because that's where I learn content. And that make more sense. As in NCLEX, we don't need to contact the doctor whenever we encounter problems w/ our patient. We should at least assess first and do something, right? :)

the answer is 4,

rationale an evisceration is the protrusion of visceral organs through the wound opening,when a evisceration occus the nurse will place sterile towels soacked in sterile saline over the extruding tissues to reduce the possiblitiy of bacterial invasion and drying.

p/s so this is the thing i get out of it, both answers are correct 1 and 4 but question asks? which will be implemented first.? what can you as a nurse do for this patient before you call the doctor? then call the doc. this is how iam approcahing this type of questions!

thats right purple, always see what you as a nurse can do before you call the doc

a client experinces a rapid and progressive dementia along with abnormal psychiatric behvaior.the doctor schedukes the pt for a brain biopsy to rule out creutfeldt-jakob disease.(cjd) which action by the nurse is most important? sata

a. notification of the operating room (or) that the client may have cjd

b.use of standard precautions while caring for client.

c. use of contant precautions while caring for this client

d.use of airborne and contact precautions while caring for this client

i would say a and b.

c and d are incorrect because cjd is not transmissible from between people by normal contact or thru environmental contamination.

im kind of hesitant on a because is that the nurse's responsibility? i like b for sure and im selecting a along with b because its sata...
