Illinois RN endorsement

Hi Everyone!

I a foreign nurse (from the Philippines), currently holding a Vermont RN license. My employer is placing me at Illinois since they have a job opening there. I need help in endorsing my VT license to Illinois. I will be very happy to hear an advise from you since my US embassy interview will be next month. I understand that I cannot work in Illinois unless I have an endorsement.

Here are my concerns:

1. FEE of $75.00 (endorsement of license & temporary permit) - do I need to include this in the packet? or does it have to be a bank to bank transaction?

2.SSN - should I place not applicable (N/A) or just leave this blank since I am still not in the US?

3. Fingerprinting (FP-NUR) - went to NBI (FBI counterpart in Manila) and had this done. The head of the fingerprinting told me that theres a lot of fingerprinting cards are being returned because of the stains in the card, how true is that? after getting this fingerprinted at the NBI, what should be the next step? I understand that I have to send the fingerprint to Ilinois state police, how do I send the payment of $34.25 to them?

4. My attorney's secretary (the one arranging my visa) keeps on insisting that copies (xerox) of CGFNS and VISA SCREEN are accepted, they have been sending copies only and is accepted by the IBON. No need to send the original copies, is that also true? My Attorney have my original CGFNS and I will need to present my original VisaScreen to the US embassy, so I am waiting until the interview is over so i can submit these forms, however, if copies are accepted, i will no longer have to wait for the interview.

I suggest you fill in the SSN number for background check purposes. Send me a PM in my email at [email protected]

If you do not have a SSN then you cannot fill it in. Several states will not process your application or issue a license without a valid SSN. Ask your agency/attorney how to proceed

Thanks for the replies souleater11 and justbeachynurse. I have spoken with the secretary of my attorney and she said that its okay to just leave it blank. SSN will be given to me 2-3 weeks after US arrival. At that point, i should report my SSN to them.

Any help with regards to my other issues such as the fees and the certificates? Thanks much! :)

payable to Department of Financial and Professional Regulation (see

Reference Sheet, Chart I).

You need to ask the BON if they will accept copies from your lawyer

It states cheque or money order therefore that is what you submit with your application

hi. i hope your still active here. can you also help me? sorry if i cannot answer any of your questions for i am also needing mine to be answered. how can i get a fingerprinting card for illinois? i've read some threads and posts here. i have sent an email to [email protected] and [email protected]. and the latter has respond, the former has not. the latter attached a fingerprinting card and stated instructions on how to process. is this legit? or should i just wait for the response of the former email 'cause it appears to me it's more legit. kindly pls share what did you do to accomplish fingerprint. thanks
