I seek the application form for Nclex

Hi everyone, I have been scouring the net for an application form for NCLEX in Arizona i cant seem to find any the old links do not work can you please help me:banghead:. And i have a question is the fingerprint card the same in all states? cause i have 5 of them my cousin used one for california, will i use the same one for arizona or Illinois? You are not applying for NCLEX but licensure to be a RN of which NCLEX is a part of that process. Application for Az is here and you need to use their fingerprint card. http://www.azbn.gov/documents/applications/App%20Packet%20Downloads/RN_LPN%20Exam%20Packet.pdf Also need CES from CGFNS and recommend full course by course if you are an international nurse

thank you very much fo rthe link, but im only looking for the initial apllication to take NCLEX. maybe its the same but my citizenship does not even appear on the list A they provided. I believed it is for for the NCLEX passers already that need their liscence

The link takes you to the application for for licensure to be a nurse in the US it is used for both US and foreign nurses and page 2 tells you what is required by Az for foreign trained nurse.

You are not looking for a NCLEX application it is licensure application
