first i just have to say thank you to all the staff, moderators and members that have helped me on me quest to pass an application:yeah:.. i still have not passed it yet since i'm looking at it everyday and making sure i have not missed anything and if i have understood the purpose of each form. my questions is that i'm a licensed nurse in the phil just this yr 2008, the illinois bon requires the ctnur form for the application when i read its contents there are only two choices on part 1 verification of examination satus its nclex_rn and nclex pn i have not taken one of those yet.. i just want to ask if anyone here has applied for illinois with a lisence in the phil is this the same form you have used? i know the one who will fill this up is the prc, but i dont want to waste their time if i have the wrong form. by the way i got it from http://www.idfpr.com. with the other forms such as fpnur and ednur which i will not probably use and the application from which is four pages. thank you very much in advance:d

same with my question which one should i check they are asking for Healthcare Profession & Science course by course report which one is it? they kind of mixed the two choices together on the application it gets confusing :Dthanks again.

another question for the CES report i have to choose the Healthcare Profession & Science course by course Report. Then the choices below the application says: Checkhhere to indicate selection. Refer to Fee insert for current year. Suzzane4 i want to ask for your advice, what do you think i should do first the CES or TOEFL? I am currently waiting for some clear answers from the people who have done this before and they are really helpful in clearing the matter to me. I am just worried that the CES will give the result directly to intercontinental testing services and not to me since i have not yet submitted an application to them(the TTS). The CES is my first step do you think its right? I chose TOEFL since it is the one required for Illinois but they also accept IELTS i think. Anyway thank you for letting me consider IELTS i did not know the difference before, I would rather talk to a personel one on one rather than a computer. thanks again hope you reply

hello everyone im going to submit to eres the application form for my course by course evaluation there is a question there where i have to put the name of the state im applying for and the address of the bon, i just want to know which address should i put in it is it: [color=#009966]mailing address springfield office: 320 west washington st springfield, il 62786 this is the mailing address of the bon or [color=#292526][color=#292526][color=#292526] continental testing services inc. p.o. box 100 lagrange, illinois 60525-0100 this is the address where all my documents for application should be sent according to the guide given to me. lastly will eres give me the evalution resukt or send it to the state im applying for? since im doing the ces and toefl first then send the application to the state to take nclex thank you very much:d

thank you very much fo rthe link, but im only looking for the initial apllication to take NCLEX. maybe its the same but my citizenship does not even appear on the list A they provided. I believed it is for for the NCLEX passers already that need their liscence

And i have a question is the fingerprint card the same in all states? cause i have 5 of them my cousin used one for california, will i use the same one for arizona or Illinois?
