How to request nclex-rn results be forwarded to ichp for visa screen certificate

Hi everyone,

Can somebody post procedures on how to forward nclex (CA) results to cgfns/ichp for visa screen certificate? Are there any fees? are they payable thru credit card or manager's check? Please advise us how. Thank you.

There is a fee $60 and the form should be in the visa screen handbook

I would like to clarify if it is necessary to wait for CGFNS to assigned ICHP number to me for my visa screen certificate application before I will proceed in sending request for verification of licenses/ academic records to the licensing agencies and schools?These forms ask for ICHP # to be filled up.Do I need to or is it just okey if even if I still dont have ICHP #?

All states require a fee to submit the results to ICHP/CGFNS. And they are all different. CA is one of the higher ones at $60 US.

Money order is the best way to submit it payable in US dollars. The form is with the application that needs to be sent to them and then they will complete it and forward it on for you.

It's ok, since this will be your first time to avail of any service from CGFNS or ICHP. Leave it blank. They are only asking if you have one to know if you previously availed one of their services before and this also helps collating every service you will avail or have availed into one file for you. They will assign you one once you availed of the VSC service. And FYI, your CGFNS and ICHP # would be the same.
