So my wedding anniversary is coming up... 3 years. I honestly never thought much of my old name still being on documents... I have a 15 year old who has my maiden name, so I get called "Mrs. (maiden name) all the time. I never even take my nursing license out of my wallet. Haven't seen it since I started my job 2 yrs ago. Time to renew, and I realized mistake. i was digging through my desk drawer and found my name change request to the board of nursing... Check and all, still in the envelope. It was a stupid mistake. I saw someone mention in an old thread that I can be charged with fraud?! I'm terrified to apply for the name change now. What do I do??

I know many nurses that practice under their maiden name. I know of a few doctors that practice under their maiden name. You are not in trouble for that.
Mass board of nursing states I am required to submit my name change within 30 days.

that is if you choose to change your name. i don't know what to tell you to do now.... boards can be funny. Do you use your "married" name at work? did you change your SS card name?

I would just send in the name change. I can't imagine that this has never happened to someone before. Highly doubt they are going to go after you.
