A lot will depend on which country you are looking at for a visa and what sort of visa it is

it would help if you state the country and what type of visa :)

It really takes long when you're doing it by your self and it depends on what type of visa you are applying for.
If you are applying for a Visit Visa for US, i know someone who gives assistance, the same person who helped me. I just acquired my 10-YEAR US VISIT VISA last April 8, 2010 but it really cost you MONEY. If you need more information, just ask me.
don't know how to make a PM at this forum.
site requirements are 15 or more posts to be able to send a pm

Doesn't matter if you have a job unless you qualify for a H1b visa and the hospital is willing to go down that route you have no choice but join the queue and wait like everyone else for a immigrant visa.
ohh..i think i just gonna go with immigrant visa
Then you will be affected by retrogression and currently have a long wait ahead of you. Has the hospital lawyers started the filing yet? If not then that is something that needs to be started

From the sound of it, this poster used "extra effort" to get a tourist visa to the US. Tsk, tsk....
