Hi there! I'm a new grad, a few months out, currently working on a med/surg floor, and I'm trying to decide on a specialty to go into. It seems like there are a lot of job listings for case management nurses and I like the idea of creating continuity of care, rather than the 'treat em and street em' mentality that seems to prevail on the floor. I was wondering how you guys feel about being in case management? Can you give me some pros/cons?

Haha! I get to do both now! I was a school nurse for four years and am now a school nurse case manager!! Only been a few weeks but it is the best of both worlds for me ?

I think it’s silly that someone wanted to comment and say that having two years of experience or less will make you be less effective in the CM position. If there is a company willing to take a chance on that person then why put them down for wanting to try? If it doesn’t work then it doesn’t work. I’ve been a LTAC Nurse for 2 years and I’ve been ready for a change and to get away from 12 hour shifts for a little while while my baby is young. I got a CM job and they’re willing to teach me so I’m ready to learn! Either I’m going to hate it or I’m going to love it but I will never know unless I try it out! Hi are you still doing case management at LTAC? How was your experience? If I am only experienced in Medsurg/Medsurg tele...will I be screwed if they assign me to a different patient population like ICU?

My husband was terminally ill with Cancer last Summer when suddenly the NCM informed me that he was going to be discharged. I was stunned. His rapid decline was ongoing, and he was a fall risk. I had no equipment at home.... and no help to care for him. I objected to this discharge and from that point on, I was considered "unkind" and non cooperative. I was simply advocating for my husband...his care and safety. We were pressured to take him home prior to our "family meeting" but we refused. I had explained that it was imperative that the "safety check" of my home should be done prior to him coming there. We managed to keep him in the hospital until after the family meeting but then we had no choice. My sons brought him home. The safety check was not done before he got home. He was on palliative care so the durable equipment he needed (as determined by his home nurse visit first day at home, AFTER he got home) would not be there for a full week. He spent his days at home in a recliner because he could not tolerate his bed. So, on his 3rd day home he collapsed...we had no wheelchair so my sons had to practically carry him out for his radiation treatments. He nearly lost consciousness. We had to call 911 and he was returned to that hospital, endured another ER boarding (28 hours). My instincts were absolutely right. He was much too sick to be discharged in the first place. So, my question is this...WHO exactly is responsible for discharging patients to home too soon.

THIS THIS THIS.....1,0000 TIMES. Although i do like case management...im getting bored out of my mind. And i dont feel like im making a difference. I work from home a d most days i find myself doing busy work. I just cant take it anymore.
