No I don't know what that it is. Is it something when u open the back ground program? I haven't done that. No need to really. I still have to do CPR and physical this week.

Wow, congrats on your A! :) I got a B!

Also quick question for everyone. On the physical form that we were given, it has a space for a 'hearing test' . I called the clinic that is posted inside the nursing office room to ask them if they do a hearing test but they said they didn't!! does anyone know where to get a 'hearing test'??

Your doctor will do that hearing test when you bring that form. He will do everything in there.

Anyone know how to get your "medical histoty " checked at? I have no idea about all the requirements such as medical history, handbook acknowledgement, CPR acknowledgment,...

I have no clue what those are! Never heard of them. I emailed Ms. January to ask but haven't had a reply back from her yet.

ok, for CPR acknowledgement just click the box and it'll give you a form that you have to print and sign, then scan. that goes the same for Essential Requirements Acknowledgement!

As for the Handbook, we will receive that in the orientation.

I don't know about medical history though. I emailed Ms. January but I didn't get a response for that. If anyone finds out what that is, please let us know.

I am very frustrated right now. Does any of you have problem with the Physical Exam form? I used the form that Ms. January e-mailed us, I had my doctor stamped it and everything. But now those Certified people said that it was not the right form. Do I have to go to the doctor again? Grhhhhhh.... Why does going to nursing school cost so much $$$$????

hello nhpl! look all the way of the bottom of the form, does it say 2010?? cus if it does, then that means you have the old version.

Ms. January sent the old version out and she didn't give me the new form until I called her to ask her about something on the form. :/ give me your email address and i'll send the new form Ms. January sent me. the "new" form she sent me is the 2012 version, but i'm just going to submit that since it's the form she specifically told me over the phone to use.

yeah for real. it's a whole load of confusion!

and it's no problem, rant away! it is really frustrating that we got sent OLD physical forms. i would have thought Ms. January sent the new forms out already!

I would call and if she doesn't accept it - I'd go above her and talk to someone else. That's B.S.. The only reason I got the new form is because berryberries sent it to me, too.

But your dr. may be able to sign or stamp it without you seeing them- seems easy enough.

Well, the "wrong" form that I got stamped was my 2nd time seeing the doctor. I did not know that we had to do that form, so the 1st time I only asked him to write me a letter. So when I knew about the form, I did call the doctor's office to just have him filled and signed it but they SAID NO, I have to see him, pay $$$ and everything... so yeah... nothing is going easy...

I called Ms. January today but no one picked up the phone. I plan to go straight to school and ask about everything, but does anyone know if she's there all the times or do I have to make an appointment? I don't want to go up there and no one is at the office.

Btw, what is her office number? Help please!
