i was given an order to run blood transfusion over 6 hours secondary to hypotension after 15min with no other s/sx of reaction. our p/p says to run over 4 hours. just wanted to see what anyone else thought about a 6 hour infusion. what happens to blood after 4 hours?
We had a 6 hour transfusion once by accident. The transfusion crossed a shift change, the patient pulled his IV out right after shift change, and the day nurse was flustered and repeatedly interrupted so she didn't notice that it ran way over the 4 hr. limit.
The 4 hr. limit, by the way, is due to the blood spontaneously growing bacteria starting at that time.
I heard about that incident for WEEKS. My manager almost had a cow, judging by the noises she made. So now on my frequent VS sheet I put, next to the sticker of the number for the unit of blood, what time it was checked out of the blood bank and highlight it. If I pass off to next shift and the thing is still running, I put in bright red on the flowsheet what time that blood has got to come down, and I say it several times in report. Of all the annoying things I do, I don't get in trouble for that one.
Once I had a supervisor order me to use the same tubing for two units, when they were ordered to run over 4 hours each. I told her to write me up for insubordination if she wanted to, but refused to do it. Then I handed her the copied policy, along with an incident report, and told her to go find me another blood tubing. :-) I didn't get in trouble for that, either.
