Looking at switching to a different hospice care plan. Anyone out there who really loves/ likes theirs? If so, where did you find it? I cant imagine there is no hospice specific diagnosis/careplanning books out there for purchase however I am not coming up with anything. Please offer some suggestions. Thank You!!!!

My company has recently implemented a standardized hospice care plan that is individualized to the patient. It's 38 pages plus, but it's great.
I couldn't find anything hospice specific prior to my company implementing one. Previously, we would spend hours writing careplans and they weren't standardized...

Many hospices are now private for-profit agencies and any standardized careplans they may have developed would be considered proprietary and copyrighted.
Just a word of caution ...

That is too bad - I truly dislike ours.

We are switching over to blank care plans and the idea is to forgo the "Nursing Diagnosis" type care plans, we are using real language and real interventions and outcomes. I think that the thinking makes sense. Pain>What is their goal for pain level>Pain medication to control pain >How effect has the current regimine been and what adjustments have needed or anticipate need to be changed.
