I just took a test for a job in HH. There was ALOT of "what does this abbreviation mean questions." I answered all of them with no problem until I saw this: LVN 1w1 2w2 1w7.

At an intermittent visit type job, it would mean LPN once a week for one week, twice a week for two weeks, and once a week for seven weeks. At least from what I've encountered. Abbreviations for the frequency of visits authorized in the plan of care.

System duplicate.

Caliotter to the rescue! ? I thought it probably meant x per week visits, but I wasn't positive. I have never seen it written quite like that before. Thanks.

That's standard formatting for visit frequencies.

It's the standard abbreviation for frequency of visits.

1w1 = 1 visit for 1 week

2w1 = 2 visits for 1 week

On our Nursing Plan of Care worksheet, we have an area where we write in our frequency, and a typical plan would be:

3wk1, 2wk2, 1wk4 Meaning for the first week you'd make 3 visits, then for the next 2 weeks you'd make 2 visits per week, then for the last 4 weeks just one visit per week.
