For all you in the high schools, do you call home for every single head injury? Or for any single injury? Do you also do accident reports for any and all injuries you see?

I call for facial injuries, anything makes me wince at first site, anything a kid was super upset about, and all hits to the head that are beyond "we bumped heads" said between giggles.

I call for anything that might raise a question or might be embellished by a dramatic kid.
IE: Hi, is this Mrs X? This is Nurse T from the school. Little Timmy is fine, there's no emergency. I wanted to call and let you know that he collided with another student in PE and got a pretty good gash to his arm. I cleaned it and bandaged it. I wanted to give you a heads up in case he was complaining of any pain to it later today.
Instead of this voicemail the next day: Nurse T!!! This is Mrs X!! Little Timmy came home and his arm is all torn up from that bully in gym class! I can't believe you didn't call and let me know!! We were at the ER all last night!!**

I did once treat a minor glue gun burn - they happen all the time with MS in the art class across the hall from me. I stock burn gel to take out the sting, clean, put a bandaid on it if necessary. I don't usually call home for this - they are that minor.
However, I did get an angry phone call once from a parent about one of these causes reaming me out for a minor burn. Kid was fine, when I checked on it the next day, kid shrugged that parent had made a big deal. That parent got added to my "call every time this kid enters my office" list. Oddly enough, now parent and I have a good relationship.
Like I have said before, you can't win sometimes. I'm working on that x-ray vision, but frankly if I get it working, I won't be working in a school - I'll be famous!

On Friday I had two kids "hit in the head". One was hit with a big foam ball in the eye and actually just wanted eye drops for the eye redness. The other one was hit with the hand of another student (by accident during PE) in the same spot, above their cheek, where my student with the concussion was hit. For both I did my usual speech of s/sx of concussion, had them repeat back, gave the handout, and called home. Both parents also talked to their student and told me thanks, but my kid seems fine. One of them threw away the concussion handout on the way out the door. I'm also going to do an accident report for each incident.
