Hi there ;-) Im working on an assignment and wondering what I can use as a nursing diagnosis, the pt has a high creatinine level, is not drinking fluids currently as they feel nauseas, and the pt also has a high urea level.

What semester are you? What care plan resource are you using? Care plans are all about the patient assesment. There is not enough information here to help you.

What are the lab values; you cant just say they are high. You have to be specific

I bet you if you answer the question "what does high creatinine/high urea level mean?", you will come up with nursing diagnoses a lot quicker than you think.
