Excess fluid volume means the person is holding too much fluid in the body, not that too much is coming out. It's easy to get that confused when you're first starting out. There is a diagnosis "Diarrhea".

I'm so confused. Is "r/t" the symptoms of the nursing diagnosis? "aeb" is what you saw during assessment?
So for example, my client had lose stools. Originally he had constipation, but after taking many meds for constipation, now he's having lose stools. So I had to change my care plan & come up with another nursing diagnosis. My nursing diagnosis is excess fluid volume. What would my "r/t" be?
